Mood Disorder Questionnaire (MDQ)
The MDQ is a relatively short (17 question) self-assessment for bipolar disorder. It can often be completed in under 5 minutes.
This instrument is designed for screening purposes only and is not to be used as a diagnostic tool. This questionnaire is used only as a starting point. It is not a substitute for a full medical evaluation. It is important to note that the MDQ is common in clinical settings, and there is some evidence to suggest it is less predictive in community settings.
Bipolar disorder is a complex illness, and an accurate, thorough diagnosis can only be made througha personal evaluation by your doctor.
